Understanding Blood Sugar

What is Fasting Blood Sugar?

Fasting blood sugar is measured after not eating or drinking (except water) for at least 8 hours. It's typically done in the morning before breakfast.

Normal Ranges (Fasting):

  • Normal: 70-99 mg/dL
  • Prediabetes: 100-125 mg/dL
  • Diabetes: 126 mg/dL or higher

What is Post-Meal Blood Sugar?

Post-meal (postprandial) blood sugar is measured 1-2 hours after eating. It shows how your body processes glucose from food.

Normal Ranges (Post-Meal):

  • Normal: Less than 140 mg/dL
  • Prediabetes: 140-199 mg/dL
  • Diabetes: 200 mg/dL or higher

Why Tracking is Important

  • Helps detect early signs of diabetes or prediabetes
  • Monitors effectiveness of diet and medication
  • Identifies patterns and triggers for high/low blood sugar
  • Enables informed discussions with healthcare providers
  • Empowers you to take control of your health

Diabetes Diagnosis

To test for Diabetes Mellitus, doctors primarily use blood tests that measure the concentration of glucose (sugar) in your system. Because blood sugar fluctuates based on what you eat and your activity level, different tests look at different "time windows" of your health.

1. HbA1c Test (The "Big Picture" Test)

As discussed, this is the most common diagnostic tool. It measures your average blood sugar over the past 2โ€“3 months.

  • Normal: Below 5.7%
  • Prediabetes: 5.7% to 6.4%
  • Diabetes: 6.5% or higher

2. Fasting Plasma Glucose (FPG) Test

This measures your blood sugar at a single point in time after you have not eaten for at least 8 to 12 hours (usually done first thing in the morning).

  • Normal: Less than 100 mg/dL
  • Prediabetes: 100 to 125 mg/dL
  • Diabetes: 126 mg/dL or higher (usually confirmed with a second test on a different day)

3. Oral Glucose Tolerance Test (OGTT)

This is a more sensitive test often used to catch prediabetes or gestational diabetes. You fast overnight, have your blood drawn, then drink a very sugary liquid. Your blood is tested again 2 hours later to see how your body handles the "sugar challenge."

  • Normal: Less than 140 mg/dL
  • Prediabetes: 140 to 199 mg/dL
  • Diabetes: 200 mg/dL or higher

4. Random Plasma Glucose Test

This test is used if you are experiencing severe symptoms (like extreme thirst or frequent urination). It can be done at any time, regardless of when you last ate.

  • Diabetes Diagnosis: A result of 200 mg/dL or higher combined with classic symptoms.

Summary of Diagnostic Criteria

Test Normal Prediabetes Diabetes
HbA1c < 5.7% 5.7% โ€“ 6.4% โ‰ฅ 6.5%
Fasting Glucose < 100 mg/dL 100 โ€“ 125 mg/dL โ‰ฅ 126 mg/dL
OGTT (2-hour) < 140 mg/dL 140 โ€“ 199 mg/dL โ‰ฅ 200 mg/dL
Random Glucose Not typically used โ‰ฅ 200 mg/dL + symptoms

Important Note:

Unless you have clear symptoms of high blood sugar, a diagnosis usually requires two abnormal results from either the same test on different days or two different types of tests performed at the same time.
